• The applicants are advised to fill in all their particulars in the Online Application form carefully as submission of wrong information may lead to rejection of application
  • Please retain a print out of your finally submitted online application (complete with Payment Details), As a proof of submission of your application to OSHB, whenever required.
  • Before final submission of the online application, Applicants must check that they have filled correct details in each field of the form. After submission of the online application form, no change/ correction / modification will be allowed under any circumstances.
  • Process of filling online application:
    The process of filling online application consists of below steps:
    Step 1 - Select the scheme details and fill applicant(s) personal details, then submit the form. It will generate a unique application no. (Non - Editable)

    Step 2 - Login using the application no. and password. And fill the required information’s like Select Payment Mode, EMD, Processing fee, Application fee etc.

    Step 3 -
    Upload the required documents ,Signature(s) & photograph(s).

    Step 4 -
    See the Preview of your application.

    Step 5 -
    Pay the total amount (EMD+ Processing fee including GST+ Application fee including GST). In case of Payment Mode NEFT/RTGS please fill the transaction details and submit.

    Step 6 -
    After completion of successful payment, then the registration slip will be generated.  Please take a print-out.

  • Signature of Applicant(s) (.Jpg Format Only) (Preferred Dimensions of Signature Are 140 X 160 Pixels.)
  • Affidavit in Prescribed Format (.pdf format only)
  • Residential Proof -Telephone Bill/ Electricity Bill/ Bank Pass Book ) (.pdf format only)
  • Payment ¬†Receipt in Support of Payment of EMD/Processing Fees/GST ¬†(in case of NEFT/RTGS Payment) (.pdf format only)
  • One cancelled cheque of the applicant (in case opting for refund through NEFT/RTGS) (.pdf format only)
  • 3 Self Attested Specimen Signature of the Applicant(s) (.pdf format only)
  • Document in Support of Claim under Specific Category (Not required for general category) (.pdf format only)
